We know you have many choices when choosing a Dentist in Hackensack, NJ so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered.

We accept cash, check, MasterCard, Visa, Discover and American Express. Financing options are available, upon approval through CareCredit and Citi Health. Unless other arrangements are made in advance, full payment is due at the time of treatment.

Dental Health Program (DHP)

Here's something to smile about! We have created an innovative dental health program. The key here is preventative and restorative dental care. Besides brushing and flossing daily, when it comes to value, regular check-ups are the best investment for a healthy mouth. We want to do our part to make optimum care available to our patients.

This is not dental insurance. This program does not have a maximum for dental treatment allowed, and there is no deductible or per-office visit fee.

The best part is you receive a 10% discount on all dental treatment during your membership. This discount could potentially save you hundreds of dollars!

*This is not dental insurance. This program cannot be used with any insurance benefits, discounts, special offers, promotions outside our office. Any part of this program not used within a year of purchase is forfeited. New programs begin on date renewal payment is received. Canceled plans due to unforeseen events will default to regular treatment fees before refunds are given. This program is for those who do not have Periodontal Disease.

All services included in this plan must be completed in our office. Any services provided outside our office is your responsibility and is not subject to any of our health programs or discounts. This program cannot be transferred to another person and benefits can only be used by the enrolled member. This program cannot be combined with any services provided prior to the date of initial sign-up. Expires one year from date payment is received.

Insurance

It is our policy to provide the best dentistry for you. To do this, it is important that we do not allow dental benefits to be a determining factor in diagnosis. Your treatment will be based upon your values and needs.

As a courtesy, we will submit claims to your dental plan carrier. Generally, dental benefits pay a percentage of each procedure up to a set yearly maximum. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract.

Your co-pay and deductible are due on the day of service. If your claim is denied or we do not receive anticipated reimbursement for the balance of your claim within 30 days, we will resubmit once. If we have no response within two weeks, we will invoice you for all unpaid balances. However, we do not guarantee any estimate, and should your dental plan pay less than expected, you are fully responsible for the balance.
